Background
The
Health Outcomes Center (HOC) was established in September
1998. It serves as a primary resource for the Grady
Health System to define issues in the delivery of health
care and provide innovative and comprehensive solutions
to optimize the delivery of high quality, cost-effective
patient care. The HOC provides expertise to measure
patients’ health status and system performance,
and to oversee improvement initiatives. The HOC provides
guidance and support in the analysis of care processes.
The Center provides support to the health system, including
medical faculty, in implementing innovative, cost-saving
disease management programs to improve patient care
and clinical outcomes.
